Volunteer Student Practice Model Application

All student models must fill out an intake document. The photo upload is required. We will be in contact if there are any issues, or we have any questions. This is our standard volunteer application. We will be in contact to schedule you a month before the next class that needs models. 

Remember, permanent makeup models arrive at 9:00 on the scheduled day. We do not allow guests or children.  

These are the available class model dates. Students start working on models their last 4 weeks of their 6-month program. We will review your document and contact you to schedule your exact date.

Upload a full-face photo with no filters or makeup. Please no expressions or posing. We need a full-face photo, front facing, good lighting and clear. This is so your technician can start working on suggested designs.

Informed Consent

A student in his or her last phase of training will be providing all services. There will be a licensed instructor on site for consulting and to provide support to the student. The services will not be provided by an instructor. Before administering permanent cosmetics, cosmetic tattoo or microblading, a cautionary notice must be furnished to the recipient by the providing facility according to Southern Nevada Health District. All permanent cosmetic tattoo services should be considered permanent in nature and never be referred to as semi-permanent even though they fade in time. They must be removed if removal is desired by with tattoo removal techniques if the recipient no longer desires the pigment. However rare complications may be, the recipients must be informed that there may be bruising, swelling, scaring, discomfort, infection, hyperpigmentation, hypopigmentation, migration, allergic reaction or specifically with eyeliner procedures, corneal abrasion. The practice of tattoo and permanent makeup is not an exact science but an art. Before and after pictures are taken of the procedure on every client. This is not optional and is required for insurance purposes. The photos are only of the procedure area and are kept in your file to track pigment retention. Post procedure instructions are to be provided to every recipient of permanent cosmetics per Southern Nevada Health District. We will be providing you detailed printed after care instructions and some after care product. There is a 24-hour cancelation policy. You must cancel or reschedule online at least 24 hours prior to your scheduled appointment. No one under the age of 18 is allowed in a tattoo of permanent cosmetic treatment room per Southern Nevada Health District. You must make arrangements for children. Client/recipient guests are not permitted in the treatment rooms. If a client brings a friend or family member, their guest is welcome to wait in the reception area. UPDATE: Due to COVID-19 restrictions, no guests are permitted. You must have an appointment to enter the facility with an approved health screening. There are no refunds on procedures or services because there is no charge for model services provided by students. Some clients/models desire to document their experience on their personal social media accounts. We do not allow audio, video or photography of staff or other guests. We understand there is a strong desire to share an exciting experience, but we must insist clients respect the privacy of staff and other guests. Client privacy is of the upmost importance. We do not share or sell any client contact information. Your information is secure.

Guarantees, Risks & Complications

Students will be providing your service. There is no guarantee that your services will result in your desired result and if you are not satisfied, there are no refunds or corrective services offered. You are agreeing to have a student practice on you and you assume all risk. You may be the first live person the student has ever worked on.
